AAMC/CDC/Fullerton Sponsored Population Health Training Webinar

Dr Bradley will lead two discussions: Emerging Models for Chronic Care and Population Health: Insights from the IOM ANDAAMC/Fullerton Population Health Webinars: Where We Are, and Where We Need to Go. The AAMC/CDC/Fullerton Sponsored Population Health Training Webinars serve as a platform for leaders on the forefront of the population health movement to share how they are incorporating population health into their practice and into their clinician training. The webinars are also a place where educators can learn about care redesign efforts underway, bridging the gaps between training/research and care redesign.The webinar is geared toward faculty teaching, or interested in teaching, population health.
