Dr. DuBard has been a leader in population health management innovation
through Community Care of North Carolina (CCNC)since 2006, where she currently serves as the chief health information officer and senior vice president for population health analytics.

Dr. DuBard will discuss:
-The value of effective use of data for every aspect of population health
management, from executive decision-making to program planning and
implementation, to panel management and direct patient care
-Primary care's opportunity and challenge to broaden our focus beyond the
individual patient encounter toward population outcomes and total cost of care.
-Focusing on "impactability" rather than risk to optimize the benefit and ROI of care team interventions
