ONE TEAM Study Design: Onco-Primary Care Networking to Support Team-Based Care

As co-PI with Dr. Kevin Oeffinger, Dr. Zullig will discuss their NCI-funded (R01) ONE TEAM study, an 18-month clustered SMART trial that aims to improve chronic disease management in cancer care. Many cancer survivors have a greater mortality risk from cardiovascular disease and its risk factors, than from their actual cancer. Unfortunately, this can stem from a disconnect between primary and oncology care teams, leading to problems in coordination and quality of care. ONE TEAM uses a multi-level, theory-driven, team-based approach that:

-Optimizes the management and outcomes of cancer patients during and after treatment
-Develops 'low-touch' interventions that can be generalized, adapted, and scaled in other health care systems.

Mohammad Shahsahebi, MD, MBA, Assistant Professor in Family Medicine and Community Health, Duke University School of Medicine, will be the discussant.

About our speaker

Dr. Leah Zullig is a health services researcher and implementation scientist. Her research interests address reducing healthcare disparities, improving cancer care delivery and quality, supporting medication adherence, and promoting cancer survivorship and chronic disease self-management. She has authored over 100 peer-reviewed publications.
