With the Affordable Care Act's focus on prevention and cost-effective care, and new community health needs assessment (CHNA) requirements for hospitals, population health has become increasingly important. National leaders in population health from A Practical Playbook will speak about how collaborative partnerships between primary care and public health can lead to improved health outcomes. Join us to hear more about A Practical Playbook, The BUILD Health Challenge, and other initiatives that are designed to facilitate and support sustainable collaboration between primary care, public health, and their partners.
