Experiencing illness or injury leads families to face a complex and often fragmented health care system. The infrastructure for documenting both health and health care make it challenging to fully understand the trajectories of health and health service use. In this presentation we will use a case example to look at different data visualization strategies that depict transitions through the health care system and recovery at individual, family, system and population levels. The story will be told only with data visualizations and we will discuss how these have identified key opportunities to improve health care quality and prevent health decline.
